Formación

• Active Physical Therapist license in good standing; NH licensure or eligibility for NH licensure.
• Graduation from an accredited physical therapy program (CAPTE) and eligibility for licensure.
• Ability to work in a long-term care/geriatric rehabilitation environment and engage with residents respectfully.
• Strong documentation skills and familiarity with standard PT assessments and modalities.
• Availability to work the 35-hour-per-week schedule and participate in a travel assignment.

• Conduct comprehensive PT evaluations for residents in a geriatric long-term care setting and establish individualized treatment plans.
• Implement therapeutic interventions to improve mobility, balance, gait, strength, and activities of daily living.
• Deliver direct patient therapy sessions, track progress, and adjust plans of care as needed.
• Document assessment findings, treatment sessions, and outcomes in the facility’s electronic health record per policy.
• Collaborate with physicians, nursing staff, and therapy colleagues to coordinate care and support discharge planning and transitions.
• Ensure adherence to safety, regulatory, and documentation standards while maintaining ethical and professional practice.
