Responsabilités

• Perform comprehensive physical therapy evaluations of residents and develop individualized treatment plans focused on functional outcomes.
• Implement evidence-based PT interventions including geriatrics-focused exercises, gait, balance, mobility, strength, and ROM.
• Monitor progress, adjust plans as needed, document outcomes, and communicate results to the interprofessional team.
• Provide patient and caregiver education, safety guidance, and discharge planning including home exercise programs.
• Collaborate with nursing, occupancy, and therapy colleagues to coordinate care and ensure quality outcomes.
• Adhere to facility policies, documentation standards, and patient safety protocols.
