Apex Home Health, Hospice & Home Care is seeking a licensed Physical Therapist to join our Skilled Nursing Facility team in California. The role focuses on therapeutic care, improving mobility and strength, and advancing patient well-being in a SNF setting.

You will provide direct patient care, follow SOPs, and maintain compliance with state and federal regulations. This full-time position offers day shifts (8 hours) with at least 40 hours per week.

Qualifications

• Must hold an active Physical Therapist license in good standing.
• Graduation from an accredited Physical Therapy program (DPT or entry-level degree).
• Minimum of 2 years of relevant experience with SNF patients.
