Mansioni

• Experience working directly with patients in a long-term care or hospital setting required
• Strong documentation and verbal communication skills required
• Must demonstrate age-appropriate care knowledge and skills for the patient population served

Responsibilities

• Work split approximately 90% outpatient and 10% inpatient
• Schedule consists of 8 or 9 hour shifts, 4 days per week
• Collaborates with speech therapists and other rehabilitation staff to coordinate patient care
• Responsibilities include evaluating patients, developing individualized treatment plans, and providing hands-on therapy such as massage and therapeutic exercise
• Maintains detailed patient records and communicates progress to providers, family members, and the care team
• Collaborates closely with physicians, nurses, and other healthcare staff to coordinate treatment
• Expected to attend in-services, continuing education, and departmental staff meetings
• Maintains professionalism and confidentiality when interacting with patients and families from diverse backgrounds
