Job summary

Affinity Home Health Care Inc. seeks a skilled Physical Therapist for a travel assignment in a Long Term Care setting. You will screen, evaluate, and rehabilitate patients with physical disorders and wound care needs, and develop individualized care plans.

Requirements include a minimum of 1 year of PT experience, LTC/SNF background, state licensure (no compact licenses), and at least one professional reference. New grads with Level II clinical experience are considered.

• Qualifications: minimum 1 year of PT experience, LTC or SNF background, state licensure (no compact licenses), and at least one professional reference. New grads with Level II clinical experience are considered.

• Work details: day shifts, 8 hours, 40 hours weekly, no weekends or on-call, with flexibility for up to 7 days of time off.
