Personal Touch Home Health Care Services Inc. seeks a Travel Long Term Care (LTC) Physical Therapist with expertise in LTC/Skilled Nursing settings.

You will provide PT services in skilled nursing facilities, assess needs, develop plans, and document progress as part of a collaborative healthcare team. The role requires an active PT license (California preferred or reciprocity), at least two years in LTC/Skilled Nursing, and current BLS.

Qualifications

• Active Physical Therapist license, preferably in California or eligible for licensure by reciprocity.
• Minimum of two years recent LTC/Skilled Nursing experience.
• Current BLS certification.
