Job description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ year of recent experience and proficiency in HCHB and OASIS.

Requirements

• Minimum of 1 year of recent, relevant Home Health PT experience required
• BLS certification required
• OASIS experience required for completing Start of Care (SOC) documentation
• Proficiency in HCHB (Homecare Homebase) required
• Active driver's license and personal transportation required
• License required if applicable
• Strong documentation and communication skills required

Responsibilities

• Home Health setting serving a geriatric patient population
• Territory covers up to a 60-mile radius from the hub office, with flexibility required across the territory
• Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM)
• Average caseload of 5-7 patients per day
• On-call rotation required approximately every 3rd to 4th weekend
• Uses HCHB EMR system
• Main responsibilities include home exercise program development and patient/family education
• Treats orthopedic, neurologic, and post-surgical conditions including post-stroke rehabilitation and spinal cord injury
• Scrubs required in any color; jeans not permitted
• iPad provided for documentation; personal cell phone use not required for work purposes
• Mileage reimbursed at the IRS standard rate
• Monday orientation required, including completion of billable modules
• Travel pairs accepted, though each traveler will be assigned to their own territory and must have independent transportation
• Timely documentation and communication are mandatory expectations of the role
• RTO requests must be submitted at the time of application
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• First-time travelers will not be considered for this position
• Local candidates accepted at the travel rate
