American Traveler, LLC. is seeking a Home Health Physical Therapist with 2+ years of recent experience and OASIS proficiency to serve a geriatric population on a day shift.

Responsibilities include home exercise programs, patient and family education, and treatment of orthopedic and neurologic conditions, with a 5–7 patient daily caseload, 8:00 AM–4:30 PM, within a 60-mile territory.

Qualificações

• OASIS experience required for Start of Care assessments.
• Proficiency in HCHB charting system.
• BLS certification required.
• License required if applicable.
• Active driver's license and reliable transportation.
• Strong documentation and communication skills.

• EMR: HCHB (Homecare Homebase) required.
• No on-call requirement, though on-call rotation may be discussed.
• Territory covers up to a 60-mile radius from the hub office.
• Local candidates accepted at the travel rate.

• Travel pairs accepted, though each traveler is assigned their own territory and must have independent transportation
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
