Job summary

American Traveler is seeking a Home Health PTA with HCHB experience to provide geriatric rehabilitation care in the field. You will treat orthopedic, neurologic and post‑surgical conditions for a geriatric population using home health protocols.

The territory covers up to 60 miles from the hub office with a 5x8 day schedule, 5–7 patient visits daily, and a 30‑point weekly productivity target. Mileage is reimbursed; travel requires reliable transportation and orienting training will be provided.

Qualifications

• Minimum 1 year of recent home health experience.
• BLS certification required.
• OASIS experience required for SOC staff.
• HCHB proficiency required.
• Active driver's license and transportation available.
• Must have valid license if applicable.
• Timely documentation and communication required.
• Hospice/Home Health checklist completion for consideration.

Responsibilities

• Home Health setting serving a geriatric patient population.
• Caseload of approximately 5-7 patients per day.
• Productivity target of 6 visits daily or 30 points weekly.
• Day shift schedule: 8:00 AM–4:30 PM.
• HCHB EMR experience required.
• Territory covers up to a 60-mile radius from the hub office.
• No on-call requirement, though rotation may be discussed.
• Provide rehabilitation care for geriatrics including ortho/ neuro/post-surgical.
• Treat hip fractures, joint replacements, spinal surgeries, strokes, TBI.
• Uses gait belts, walkers, crutches, prosthetics; scrubs required.
• Mileage reimbursed at IRS rate; iPad provided for docs.
• Orientation and billable training modules required on start.
• Local candidates accepted at travel rate; travelers must maintain territory.

• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in HCHB required
• Active driver's license and personal transportation required
• License required if applicable
• Timely documentation and communication required
• Hospice/Home Health checklist required for consideration

Responsibilities

• Home Health setting serving a geriatric patient population
• Caseload of approximately 5-7 patients per day
• Productivity expectation of 6 daily visits or a minimum of 30 points weekly
• Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
• HCHB (Homecare Homebase) EMR experience required
• Territory covers up to a 60-mile radius from the hub office, with occasional additional distance based on patient needs
• No on-call requirement, though rotation may be discussed during interview
• Provide rehabilitation care for geriatric patients including orthopedic, neurologic, and post-surgical conditions
• Conditions treated include hip fractures, joint replacements, spinal surgeries, stroke recovery, and traumatic brain injury
• Utilizes equipment such as gait belts, walkers, crutches, and upper/lower extremity prosthetics
• Scrubs required in any color; no jeans permitted
• No cell phone provided; iPad provided for documentation
• Mileage reimbursed at the IRS standard rate
• Monday orientation required, including completion of billable training modules
• First-time travelers will not be considered for this role
• Travel pairs accepted, though each traveler must maintain their own territory and transportation
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Requested time off (RTO) must be submitted at the time of application
• Local candidates accepted at the travel rate
• Traveler must remain flexible with territory assignments based on patient needs
