American Traveler, LLC. is seeking a Home Health Physical Therapist with at least 1 year of recent experience and HCHB EMR proficiency to cover a geriatric caseload. The role requires travel within up to a 60-mile radius, with an average caseload of 5–7 patients per day and a 8:00 AM–5:00 PM schedule.

Applicants must have a BLS certification, OASIS experience, and an active driver’s license. On-call weekends and travel experience are required, and IRS mileage is reimbursed.

Qualifications

• Minimum 1 year of recent home health PT experience required.
• BLS certification required.
• OASIS experience required for Start of Care documentation.
• Proficiency in HCHB EMR required.
• Active driver's license and personal transportation required.
• License required if applicable.
• Timely documentation and communication required.
• Prior travel experience required; first-time travelers not accepted.

Responsabilités

• Home health setting treating geriatric patients.
• Average caseload of 5-7 patients per day.
• Day shift schedule, 5x8 hours from 8:00 AM to 5:00 PM.
• On-call rotation required approximately every 3rd to 4th weekend.
• Territory covers up to a 60-mile radius from the hub office.
• Traveler must be flexible with territory assignments.
• iPad provided for documentation; mileage reimbursed at IRS standard rate.
• Responsibilities include home health physical therapy evaluation and treatment for geriatric patients.
• Monday orientation includes completion of billable modules.
• Scrubs required in any color; no jeans permitted.
• Cell phone use not permitted; iPad provided for work purposes.
• RTO must be submitted at time of application.
• Local candidates accepted at the travel rate.
• Travel pairs accepted, though each traveler is assigned their own territory and must have separate transportation.
• Former perm employees must be separated for at least 6 months and approved by HR before returning as a traveler.
