Job summary

American Traveler, LLC. is seeking a Home Health Physical Therapist with 2+ years of experience and valid state licensure to provide skilled home visits to a diverse patient population in Florida.

Responsibilities include conducting assessments, developing care plans, educating patients and families, and maintaining documentation using BayCare and Cerner systems. Travel rate applies for eligible candidates, with a typical 5x8 schedule.

Qualifications

• Minimum 2 years of home health experience.
• BLS certification required.
• State license required if applicable.
• Valid driver's license and reliable vehicle for home visits.
• Home health experience required; OASIS training preferred.
• Experience with cardiopulmonary, orthopedic, and neurologic populations.
• Proficiency with home health equipment (crutches, gait belts, walkers, slings, etc.).
• Skilled in assessment, documentation, home exercise programs, and patient/family education.
• Knowledge of signs of abuse (physical, sexual, psychological).
• Strong safety and injury prevention knowledge.

Responsibilities

• Provide skilled home visits to patients with cardiopulmonary, geriatric, neuro, orthopedic, and oncology conditions.
• 5x8 day shift, 8:00 AM – 5:00 PM.
• Caseload of 6–8 visits per day (min 6 points per day).
• Open to every 3rd weekend; self-scheduling of visits.
• Utilize laptop, MiFi, and portable printer for documentation.
• Document via BayCare Forms, BayCare Scheduling, and Ace Timesheet.
• Cerner experience preferred.
• Locals welcome at the same pay rate.
• Conduct skilled visits, assess patients, develop/update care plans, educate families.
• Wear solid color scrubs; COVID vaccination or exemption on file.

American Traveler is seeking a Home Health Physical Therapist with 2+ years of experience and valid state licensure to provide skilled home visits to a diverse patient population.

Requirements

• Valid driver's license and reliable vehicle required for home visits
• Home health experience required; OASIS training preferred
• Experience with cardiopulmonary, orthopedic, and neurologic patient populations required
• Proficiency with home health equipment including crutches, gait belts, walkers, slings, sliding boards, collars, and supplemental oxygen delivery
• Skilled in assessment, documentation, home exercise programs, and patient/family education
• Knowledge of signs and symptoms of physical, sexual, and psychological abuse
• Strong understanding of safety and injury prevention principles

Responsibilities

• Home health setting providing skilled visits to patients with cardiopulmonary, geriatric, neuro, orthopedic, and oncology conditions
• Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
• Caseload of 6-8 home visits per day, with a minimum of 6 points per day (may increase at manager's discretion)
• Every 3rd weekend required
• Self-scheduling of visits and hours based on office needs
• Uses laptop, MiFi, and portable printer for documentation and connectivity
• Documentation completed via BayCare Forms, BayCare Scheduling, and Ace Timesheet systems
• Cerner experience preferred
• Locals accepted at the same pay rate
• Responsibilities include conducting skilled home visits, patient assessments, developing and updating care plans, and educating patients and families
• Must wear solid color scrubs of any color
• Must be COVID vaccinated or have an exemption form on file
• Must have flu shot or signed declination; mandatory mask wearing required if declined
• Candidates must reside more than 100 miles from the facility to qualify for travel rate
• Open to first-time travelers
• Former BayCare employees must wait at least 12 months before returning as a traveler
• RTO is not accepted for this contract
