Job summary

American Traveler is seeking a licensed Physical Therapist for a Home Health assignment in Florida. Requires 1+ year of home health experience and active state licensure; BLS is required; OASIS/Cerner experience preferred; valid driver's license and reliable vehicle for home visits.

The position involves 6-8 home visits per day, day shift (8:00 AM–5:00 PM), serving Medicare, Medicare Advantage and commercial patients.

Qualifications

• Cerner experience preferred.
• Must have a valid driver's license and reliable vehicle for home visits.
• Proficiency with equipment (gait belts, walkers, lifts, boards, collars, wheelchairs).
• Skilled in ortho, neuro, cardiopulmonary and documentation (evaluations, care plans, OASIS).
• COVID vaccinated or exemption on file; flu vaccination or declination with mask if declined.

Responsibilities

• Home Health visits for Medicare, Medicare Advantage, and commercial patients.
• Day shift 8:00 AM – 5:00 PM; 6-8 visits per day; 1-2 weekend shifts.
• Perform patient assessments, treatment plans, and home exercise programs.
• Provide medication education and family/caregiver education.
• Document with BayCare forms, BayCare Scheduling, and Ace Timesheet.
• Recognize and report signs of abuse; ensure safety and injury prevention.

• Proficiency with equipment including gait belts, walkers, sit-to-stand lifts, sliding boards, cervical collars, and wheelchairs required
• Skilled in ortho, neuro, cardiopulmonary, and documentation practices including evaluations, care plans, and OASIS assessments
• Must be COVID vaccinated or hold an exemption on file
• Flu shot or declination required, with mandatory mask wearing at all times while at work if declined

Responsibilities

• Home Health setting providing skilled visits for Medicare, Medicare Advantage, and commercial insurance patients
• Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM)
• Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL-focused patients
• 6-8 home skilled visits per day, with a minimum of 6 points per day (may increase at manager's discretion)
• 1-2 weekend shifts required per contract
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems
• Laptop, MiFi, and portable printer provided for documentation and connectivity
• Responsibilities include patient assessment, treatment planning, home exercise programs, medication education, and family/caregiver education
• Must recognize and report signs of physical, sexual, or psychological abuse
• Focus on safety and injury prevention during home visits
• Any solid color scrub permitted
• Locals accepted at the same pay rate
• Open to candidates without previous travel experience
