Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans to restore and improve physical function
- Deliver therapy interventions using evidence-based techniques and established protocols
- Collaborate with an interdisciplinary team and participate as a co-leader in patient group activities
- Complete accurate clinical documentation within established deadlines; communicate discharge plans to the care team
- Supervise assigned assistants and provide patient education on safety and self-management
- Maintain EPIC electronic health record documentation and ensure continuity of care across transitions
Requirements
- Doctor of Physical Therapy (DPT) degree; required, if license was obtained on or after January 2016, or foreign equivalent
- Physical Therapy licensure in the state of practice (California) required
- CPR/BLS certification required
- At least two years of recent physical therapy experience
- Pelvic floor therapy experience with PRPC or WCS certification, or actively pursuing certification (preferred)
- EPIC experience required
- Current nursing license per the state of assignment (as referenced in job notes)
