Responsibilities
- Conduct patient evaluations and develop individualized treatment plans to restore and optimize physical function
- Provide physical therapy interventions using appropriate techniques and protocols; adjust plans based on progress
- Document assessments, treatment plans, progress notes, and discharge plans within established deadlines
- Collaborate with an interdisciplinary team and participate in group or patient education sessions as needed
- Supervise assigned assistants and coordinate care delivery within the patient’s care plan
- Support discharge planning and communicate plans to the patient and care team
Requirements
- Doctor of Physical Therapy (DPT) degree; required; applicable when licensure was obtained on or after January 2016 or foreign equivalent
- Physical Therapy licensure in the state of practice (California) or eligibility to practice
- CPR certification or Basic Life Support (BLS) from an approved vendor
- Valid driver’s license and at least 21 years of age
- Minimum 1 year of home health experience
- Knowledge of HCHB and OASIS; EPIC experience preferred
