Responsibilities

• Plan and conduct segments of outpatient physical therapy treatment programs to restore and improve physical, social, and mental functions while aligning with department objectives.
• Serve as co-leader in an interdisciplinary approach to patient groups, coordinating with team members to optimize recovery.
• Assess patient status, establish treatment goals, implement plans of care, and monitor progress toward functional outcomes.
• Document evaluations, treatments, progress notes, and outcomes in the medical record with accuracy and timeliness.
• Educate patients and families on home exercise programs, safety considerations, and activity modifications.
• Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate care and adjust plans as needed.
