Responsibilities include evaluating patients, creating and updating treatment plans, documenting progress, and collaborating with the care team. Requires DPT and CA licensure, BLS/CPR, and clean driving record; hospital-based practice preferred.

Physical Therapist, General — Clearlake, CA. Travel - Temporary Assignment starting 09/28/2026 for 13 weeks. Weekly pay range: $2,615–$2,693 per week with 40 guaranteed hours. Shift: Day shift, 5x8 (Mon–Fri). Setting: a 25-bed critical access hospital.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans to restore and improve physical function
• Implement evidence-based PT interventions and adjust plans based on patient progress
• Complete comprehensive documentation, including assessment findings, treatment plans, progress notes, and discharge summaries, within established deadlines
• Collaborate with an interdisciplinary care team and co-lead patient groups as appropriate
• Supervise assigned therapy assistants and communicate discharge or follow-up needs to patients, families, and care team

Requirements

• Doctor of Physical Therapy (DPT) degree (or foreign equivalent) when applicable; license in the state(s) of practice
• Current physical therapy licensure in the state(s) of practice and eligibility to practice in California for this assignment
• CPR certification or BLS (American Heart Association) required; additional BLS/CPR variants accepted per policy
• Valid Driver’s License and must be at least 21 years of age
• Must know HCHB and OASIS; EPIC experience; at least two years of recent PT experience; 1 year of home health experience preferred
• Ability to work in a hospital-based setting; no weekends; day shift; 40 hours per week
