Job summary

HealthForce Healthcare seeks a Physical Therapist (General) for a travel assignment in Clearlake, CA. The 13-week temporary role starts 09/28/2026 with 40 hours per week, day shift (Mon–Fri) and a 25-bed critical access hospital setting.

Responsibilities include evaluating patients, creating and updating treatment plans, documenting progress, and collaborating with the care team. Requires DPT and CA licensure, BLS/CPR, and clean driving record; hospital-based practice preferred.

Qualifications

• DPT degree required with CA license.
• Current physical therapy licensure in CA for assignment.
• CPR/BLS certification required.
• Valid driver's license and at least 21 years old.
• Experience with HCHB, OASIS, and EPIC is preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Implement PT interventions and adjust plans based on progress.
• Document assessments, plans, progress notes and discharge summaries timely.
• Collaborate with interdisciplinary team and co-lead patient groups.
• Supervise therapy assistants and communicate discharge needs.
