Job summary

Personal Touch Home Health Care Services Inc. is seeking a Physical Therapist (PT) to assess referred patients requiring rehabilitation, develop individualized treatment plans, and deliver skilled PT interventions according to physician orders.

Job description

Role: The Physical Therapist (PT) is responsible for assessing referred patients needing rehabilitation services, developing and implementing individualized treatment plans, and providing skilled physical therapy interventions in accordance with physician orders.

• Key Duties: Evaluate patient conditions, set realistic goals, document progress, educate patients and families, and collaborate with multidisciplinary teams to ensure effective care.
• Qualifications: Must hold a Doctorate or Bachelor's degree in Physical Therapy, pass the NPTE, possess current licensure or be in the process, and have at least one year of experience. CPR certification is required.
• Skills: Strong assessment, documentation, and communication skills, ability to develop tailored treatment plans, and teamwork capabilities.
