The Judge Group is actively seeking an experienced Home Health Physical Therapist (PT) in Ohio to join our healthcare team. The PT will deliver in-home care, develop personalized treatment plans, and focus on improving mobility, balance and safety for patients in their homes.

Ideal candidates will hold an Ohio PT license, have at least 1 year of home health experience, and demonstrate strong assessment, communication, and patient education skills.

Qualifications

• Current Physical Therapist (PT) license in Ohio.
• Minimum of 1 year of Home Health Physical Therapy experience.
• Strong clinical assessment and treatment skills.

Responsibilities

• Provide skilled physical therapy services in patients' homes according to the plan of care.

Location/Coverage Area: Highland, Clinton, and Fayette Counties, OH

Pay: $50.00/hourly

Position Overview

We are seeking an experienced Home Health Physical Therapist (PT) to join our team and provide exceptional patient care in the home setting. The Physical Therapist will develop and implement individualized treatment plans designed to improve mobility, increase strength, enhance balance, and promote patient safety and independence.

Schedule & Productivity

• Hours: 40 hours per week


• Daily Schedule: Monday through Friday, 8:00 AM - 4:30 PM


• Productivity Expectation: 30 points per week


• On-Call/Weekend Requirement: None

Key Responsibilities

• Provide skilled physical therapy services in patients' homes according to the established plan of care.


• Perform gait and balance training to improve patient mobility and reduce fall risk.


• Implement strengthening and therapeutic exercise programs tailored to individual patient needs.


• Conduct home safety assessments and provide recommendations to enhance patient independence and safety.

Required Qualifications

Preferred Qualifications

• Experience with Homecare Homebase (HCHB) documentation software.


• Ability to work independently while managing a patient caseload effectively.
