Job summary

Northshore Home Health Care is seeking an Outpatient Physical Therapist to travel between facilities, helping patients recover from injuries and manage chronic conditions in outpatient settings.

You will deliver patient-centered therapy, tailor treatment plans, and collaborate with healthcare teams while experiencing diverse locations. A PT degree, active state license, and BLS certification may be required.
