• Assess residents’ OT needs and establish individualized treatment goals within the OT framework.
• Plan and implement therapeutic interventions to improve activities of daily living, cognitive function, motor skills, and safety in daily tasks.
• Monitor progress, adjust treatment plans as needed, and document outcomes clearly in charts and progress notes.
• Collaborate with an interdisciplinary team (nursing, physical therapy, speech-language pathology, social work) to coordinate care and optimize resident outcomes.
• Educate residents and families on home programs, safety strategies, and community-based care transitions; participate in discharge planning as appropriate.
• Ensure compliance with regulatory standards, facility policies, and documentation requirements for OT services.

Requirements

• Active, unrestricted Occupational Therapy license to practice in Washington state (or eligibility to obtain per state regulations).
• Graduation from an accredited OT program (degree appropriate to current licensure requirements).
• Experience in geriatric rehabilitation or skilled nursing facility settings preferred.
• Strong clinical documentation, communication, and collaboration skills.
