Responsibilities
- - Conduct occupational therapy evaluations and develop individualized treatment plans for residents in a long-term care setting
- - Implement and monitor therapies aimed at improving activities of daily living, mobility, reach and strength, and cognitive/visual-moments as appropriate
- - Utilize appropriate modalities, adaptive equipment, splinting, and environmental adaptations; document progress in resident records
- - Collaborate with the interdisciplinary team (nursing, therapy, social work, activities) to coordinate care and support discharge planning
- - Manage a cross-site schedule to ensure continuity of care across two facilities, including targeted visits for residents on both sites
- - Maintain compliant documentation and adhere to regulatory standards and facility policies
Requirements
- - Active Occupational Therapist license for the state of Washington, or eligibility to obtain licensure
- - Graduation from an accredited OT program; NBCOT certification preferred or eligible- Ability to work in a long-term care setting and manage cross-site coverage
- - Excellent communication and documentation skills; ability to collaborate with an interdisciplinary team
- - Minimum educational background: degree in Occupational Therapy (bachelor’s or higher as required by state)
- - Willingness to travel between two facilities within the assignment and maintain a consistent schedule
