Responsibilities

• Conduct comprehensive occupational therapy evaluations for a geriatric population and develop individualized treatment plans aimed at improving activities of daily living, motor skills, cognition, and safety.
• Implement evidence-based OT interventions to restore, improve, or maintain client function and independence.
• Reassess progress regularly, document outcomes, and adjust treatment plans accordingly.
• Collaborate with an interdisciplinary care team to coordinate care, modify plans, and support discharge planning.
• Educate clients and families on home programs, safety strategies, and equipment needs; provide training on adaptive devices when appropriate.
• Maintain accurate and timely documentation in the electronic health record and adhere to regulatory and facility requirements.
