Responsibilities

• Conduct comprehensive occupational therapy evaluations for residents and develop individualized treatment plans addressing ADLs, mobility, upper and lower extremity function, and functional independence
• Deliver evidence-based therapy through one-on-one sessions and group activities, monitoring progress and adapting plans to maximize outcomes
• Document clinical findings and progress in the resident’s medical records per facility policy and regulatory requirements
• Collaborate with an interdisciplinary team (nursing, physical therapy, speech-language pathology, social work) to coordinate care and support discharge planning
• Educate residents and families on treatment goals, home programs, and safety strategies to support ongoing recovery and independence
• Maintain safety and infection control standards, comply with state and federal regulations, and participate in quality improvement initiatives as applicable
