Job summary

• Charting System: Epic is required.
• Travel experience is required; can screen 1st time traveler.
• CA state license required; pending licenses accepted but must be licensed on start date.
• Onsite day shift; 8 hours/day, 5 days/week; start ASAP or 11/02/2026.

Responsibilities

• Care coordination and discharge planning per admission criteria.
• Determine Medical Necessity per Evidence-Based Guidelines.
• Disease management and Needs Assessment/Order DME; Plan of Care.
• Regulatory compliance with CMS, CPT coding, HIPAA guidelines.
