Qualifications
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted Nurse Practitioner license in the state of residence or ability to obtain by start date.
- Board certified through AANP or ANCC with Family/Adult/Gerontology options.
- For PAs: Graduate of an accredited Physician Assistant degree program and active license.
- Current DEA licensure/prescriptive authority or ability to obtain post-hire.
- Ability to complete physical requirements and travel to facilities in OR.
- Driver's license and reliable transportation.
Responsibilities
- Conduct comprehensive assessments.
- Manage members through acute medical situations or chronic exacerbations.
- Support short-term sub-acute rehabilitation episodes.
- Ensure accurate documentation including ICD-10.
- Collaborate with interdisciplinary care team.
- Engage in advanced illness and end-of-life discussions.
- Counsel patients and caregivers.
