Job summary

Your Health Organization in Georgia is seeking a Registered Nurse to join our growing primary care team as a Nurse Case Manager. You will travel to patients' homes and facilities to evaluate patient needs, initiate telemedicine visits, and coordinate care with providers and families.

The role emphasizes strong communication, record review, and care-planning to ensure quality, compliant care across home, facility, and clinic settings.

Qualifications

• Must be a licensed nurse (RN preferred).
• Three+ years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to collaborate with interdisciplinary care teams.

Responsibilities

• Facilitate receiving medical records from providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management when eligible.
• Initiate Care Management Plans of Care when appropriate.
• Coordinate with patient health care team and facilities.
• Travel to patients' homes and care facilities as required.
