Job summary

Your Health Organization is seeking a Registered Nurse to join our growing primary care team in the Macon area. The Nurse Case Manager will travel to patients’ homes and facilities, coordinating with providers and initiating telemedicine visits as needed.

Responsibilities include reviewing records, enrolling patients in Care Management, and developing Care Plans. Strong communication and leadership skills, plus the ability to work flexible hours and travel daily, are essential.

Qualifications

• Must be a licensed nurse in good standing.
• 3+ years clinical experience preferred.
• Experience in community settings preferred.

Responsibilities

• Facilitate receiving and review medical records.
• Enroll patients in Care Management when eligible.
• Initiate Care Plans and coordinate with care team.
• Travel to patient homes, facilities, and support telemedicine visits.
• Document Care Management activities and maintain care plans.
