Job summary

Your Health Organization in the Athens area is seeking a Registered Nurse to serve as a Visiting Nurse (Nurse Case Manager). You will travel to patients' homes and long-term care facilities, coordinate with providers, and initiate telemedicine visits to support quality care.

This full-time position features 8-hour day shifts, travel required daily, and responsibilities to manage care plans, documentation, and collaboration with interdisciplinary teams.

Qualifications

• Licensed Registered Nurse in good standing.
• Minimum 3 years of clinical experience.
• Experience in community settings preferred.
• Strong communication and collaboration with care teams.
• Good written and verbal skills.
• Basic computer skills.
• Ability to lead and manage multiple responsibilities.
• Willingness to travel between offices/facilities.

Responsibilities

• Facilitate collection of medical records from providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care when eligible.
• Document Care Management activities accurately.
• Coordinate care with interdisciplinary teams.
• Evaluate for home health, hospice, RPM devices as needed.
• Travel to patients' homes and facilities; on-call may be required.
