Job summary

Your Health Organization in Georgia is seeking a Registered Nurse for a Nurse Case Manager role serving patients across the Macon area. The role involves home visits, coordinating with providers, and initiating telemedicine visits to support quality care.

Travel is required daily to facilities and patient homes. The position is full-time, 8-hour shifts, Monday–Friday, 8am–5pm, with opportunity for growth and comprehensive benefits.

Qualifications

• Must be a licensed nurse in good standing.
• Three years clinical experience preferred.
• Experience in community settings preferred.
• Strong written and verbal communication.

Responsibilities

• Facilitate receiving medical records from providers.
• Review medical records and complete consents.
• Enroll patients in Care Management when eligible.
• Initiate Care Management Plan of Care when eligible.
• Capture diagnoses with specificity and resolve gaps.
• Complete AWV’s for provider review.
• Perform cognitive impairment screenings.
• Conduct SDoH assessments and screenings.
• Complete ACPs with patient and provider.
• Evaluate for home health, hospice, RPM options.
• Coordinate with care team and patients.
• Log Care Management activities accurately.
