Job summary

Trilogy Health Services is seeking a Travel Licensed Practical Nurse to support short- and long-term assignments across Indiana, Ohio, Michigan, and Kentucky, delivering direct, quality clinical care while supervising nursing assistants on the unit.

This role requires adaptability, a current state LPN license and CPR certification, and a valid driver's license to cover multiple facilities.

Qualifications

• Must hold a current, valid state LPN license.
• Current CPR certification.
• 3–5 years of relevant nursing experience preferred.
• Valid and unencumbered driver's license.

Responsibilities

• Visit residents daily to assess physical and emotional status and notify the charge nurse, physician, or Director of Nursing of changes.
• Identify problems and emergencies and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care within the LPN scope.
• Monitor medications, medical supplies, and equipment to ensure adequate availability.
• Perform administrative duties such as completing forms, reports, evaluations, and charting.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policy.
