Job summary

Trilogy Health Services is seeking a Travel Licensed Practical Nurse to provide direct clinical care and supervise nursing assistants across Indiana, Ohio, Michigan, and Kentucky. The role emphasizes adaptability while maintaining consistent standards of care on assigned units.

Travel assignments require daily resident assessment, timely communication with charge nurses and physicians, and strict adherence to facility policies.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.
• 3-5 years of relevant experience preferred.
• Valid and unencumbered driver's license required.

Responsibilities

• Visit residents daily to assess status and notify nurse/physician if changes observed.
• Identify problems and emergencies; initiate lifesaving measures when needed.
• Provide holistic care within LPN scope.
• Monitor medications, supplies and equipment inventory.
• Complete medical forms, reports and charting as necessary.
• Report occupational exposures per facility policies.
