Job summary

Trilogy Health Services is seeking a Travel Licensed Practical Nurse to support short- and long-term assignments across Indiana, Ohio, Michigan, and Kentucky. You will provide direct clinical care and supervise nursing assistants on assigned units, ensuring consistent standards of care.

The role requires an active LPN license, CPR certification, and 3–5 years of relevant experience, plus a valid driver’s license. Travel reimbursement and a comprehensive benefits package accompany this position.

Qualifications

• Active LPN license in the state of practice.
• CPR certification required and current.
• 3–5 years of relevant nursing experience preferred.
• Valid, unencumbered driver's license required.

Responsibilities

• Visit residents daily to observe status and notify charge nurse or physician of changes.
• Identify problems and emergency situations and initiate lifesaving measures when needed.
• Provide holistic care within the LPN scope of practice.
• Monitor medications, supplies, and equipment inventory.
• Complete medical forms, reports, and charting as needed.
• Report exposure incidents per facility policies.
