HealthForce Healthcare seeks an Licensed Practical Nurse (LPN) for a travel assignment in Concord, New Hampshire. The position starts 11/09/2026 for an estimated 15 weeks with guaranteed 36 hours per week, night shift 7:00 p.m. – 7:00 a.m.

Formación

• Active LPN license in NH (in good standing).
• Ability to work night shifts in LTC setting (7:00 p.m.–7:00 a.m.).
• Graduation from an approved practical nursing program.
• Knowledge of long-term care practices and regulatory requirements.

Responsabilidades

• Administer prescribed medications and treatments per physician orders and facility policies.
• Monitor residents’ health status, respond to changes, and elevate to the supervising RN as needed.
• Provide and document routine nursing care, including ADLs, vital signs, and wound/dressing care.
• Assist with admissions, transfers, and discharges following established procedures.
• Ensure infection control standards, patient safety, and confidentiality in all care activities.
• Maintain accurate documentation in the resident’s chart and electronic health record.
