Job summary

Healthforce is seeking a Licensed Practical Nurse (LPN) for a travel assignment in a long-term care facility in Rochester, NH. The role starts on 11/02/2026 and runs for six weeks, with guaranteed 40 hours per week and evening coverage from 3:00 PM to 11:30 PM.

Local candidates only; weekend coverage is required, with potential extra hours on a 3:00 PM–11:00 PM shift. Housing assistance and fast credentialing are available through Healthforce.

Qualifications

Licensed Practical Nurse (LPN) with current licensure in New Hampshire.
Graduation from an approved practical nursing program.
Ability to work evenings and fulfill local assignment requirements; weekend coverage as scheduled.

Responsibilities

Provide direct nursing care to residents per care plans and orders.
Administer medications and treatments; document in the medical record.
Monitor vital signs, assess changes, and report to supervising nurse promptly.
Assist residents with activities of daily living and maintain a safe environment.
Collaborate with the interdisciplinary team to update care plans.
Ensure infection control practices, safety protocols, and documentation standards.
