Job summary

HealthForce Healthcare is seeking a Licensed Practical Nurse for a travel assignment in Belen, New Mexico. The position is a 12-week, 36-hour per week night shift (3x12s) at a skilled nursing facility, starting 11/01/2026.

You will assess residents, administer medications, document care plans, and collaborate with the nursing team. The ideal candidate holds an active LPN license in New Mexico, has at least one year of LPN experience in long-term care, and is willing to travel for the duration of

Active LPN license in New Mexico.
At least 1 year of LPN experience in long-term care or skilled nursing.
Ability to work night shift and travel for a 12-week assignment.

Responsibilities

Provide direct nursing care to residents during night shifts, including assessments and monitoring.
Administer medications and document accurately per care plans.
Perform routine procedures and respond to changes in residents' conditions.
Collaborate with the charge nurse and interdisciplinary team to implement and adjust care plans.
Maintain safety and infection control standards; document in the electronic health record.
Communicate changes in patient status promptly to the RN and initiate escalation when needed.
