Job summary

Healthforce in Springfield, VT is seeking a Licensed Practical Nurse for a travel assignment in a long-term care facility. The position offers a 40-hour guaranteed week with evening and/or overnight shifts for several weeks, starting 11/16/2026.

You will assess residents, administer prescribed medications, assist with daily living activities, and communicate changes to the care team. Travel coordination and assignment support will be discussed during the hiring process.

Qualifications

• Active LPN license in Vermont (or eligibility to practice)
• Experience or competency in a long-term care setting
• Availability for evening and/or overnight shifts
• Ability to fulfill 40-hour weekly travel schedule

Responsibilities

• Assess residents’ health status and monitor changes; document findings accurately.
• Administer prescribed medications and treatments; observe for adverse reactions and document responses.
• Provide direct patient care including hygiene, mobility assistance, and activities of daily living.
• Communicate changes in patient condition to the supervising RN and care team; contribute to care planning.
• Maintain thorough, compliant charting and medication administration records.
• Adhere to infection control, safety, and facility policies; participate in quality and safety initiatives.
