Job summary

Healthforce is seeking an experienced Travel Licensed Practical Nurse (LPN) for a contract assignment in Bennington, VT. You will deliver person-centered chronic disease management, routine assessments, and direct nursing care to residents in a skilled nursing and long-term care setting, with evening or overnight shifts.

This role requires an active VT LPN license or multi-state eligibility, 1+ year SNF/LTC experience, and strong collaboration with CNAs, therapists, and families.

Qualifications

• Graduate of an accredited practical nursing program.
• Active, unencumbered Vermont LPN license (or multi-state license eligible for VT).
• Minimum of 1+ year of direct nursing experience in SNF/LTC/sub-acute rehab.
• Availability for evening and/or overnight (NOC) shifts, with weekends as needed.

Responsibilities

• Medication administration & treatments as ordered.
• Resident assessment & monitoring of vital signs and condition changes.
• Interdisciplinary care with CNAs, therapists, and social services.
• Clinical documentation in EHR in real-time following regulations.
• Shift leadership to manage floor flow during evenings/overnights.
