Job summary

Healthforce seeks an experienced Travel Licensed Practical Nurse (LPN) for a contract assignment in Bennington, VT. The role focuses on chronic disease management, routine assessments, and direct nursing care in a skilled nursing/long-term care environment.

You will deliver person-centered care, communicate with families, and collaborate with an interdisciplinary team during evening or overnight shifts. Prior travel nursing experience is a plus.

Qualifications

• Graduate of an accredited practical nursing program.
• Active Vermont LPN license or multi-state license eligible for VT.
• 1+ year of direct nursing experience in SNF/LTC/sub-acute rehab.

Responsibilities

• Administer prescribed medications and perform routine skin/dressing care as ordered.
• Monitor vital signs and changes in patient status; report to RNs/doctors/families as needed.
• Collaborate with CNAs, PT/OT, and social services to follow care plans.
• Maintain accurate, real-time EHR documentation per regulations.
• Oversee floor flow during evening/NOC shifts and ensure smooth handovers.
