Job summary

Healthforce is seeking an experienced Travel Licensed Practical Nurse (LPN) for a contract assignment in Bennington, VT. You will deliver person-centered chronic disease management, routine assessments, and direct nursing care in a skilled nursing and long-term care setting, with evening and/or overnight shifts.

Qualifications

• Graduate of an accredited practical nursing program.
• Active Vermont LPN license or valid multi-state eligibility for VT.
• Minimum 1 year of direct SNF/LTC experience.
• Flexibility to work evenings and/or overnight shifts, including weekends.

Responsibilities

• Administer medications and treatments per physician orders.
• Monitor vital signs and report changes promptly to RNs and physicians.
• Collaborate with CNAs and therapists to support care plans.
• Maintain accurate real-time documentation in the EHR.
• Oversee floor flow during evening or overnight shifts.
