Healthforce is seeking an LPN for a 21-week travel assignment in Camden, Maine. The role offers three 12-hour day shifts (06:45-19:15) with guaranteed 36 hours per week across a long-term care facility. Start date is November 2, 2026, with travel support and credentialing assistance provided.

The position emphasizes direct patient care, medication administration, accurate documentation, and collaboration with the care team to ensure safety and quality outcomes.

Formación

• Active LPN license is required.
• Excellent communication and teamwork abilities.

• Proficiency with nursing documentation and EHRs.

Responsabilidades

• Provide direct patient care to long-term care residents under supervision.
• Monitor vitals, intake/output, neurovascular checks and report changes.
• Administer medications and document administration; verify MARs and participate in med reconciliation.
• Assist with ADLs and promote dignity and independence.
• Maintain nursing notes, care plans, and progress notes; update as needed.
• Collaborate with CNAs, RNs, physicians and therapy staff to coordinate care.
