Job summary

Sweetwater Care in Arlington, VA is seeking an experienced LVN/LPN to provide nursing care within scope of practice, in alignment with physician orders and center policies.

Requirements include long-term care experience, Point Click Care charting, an active LVN/LPN license, BLS/CPR certification, and preferably some nursing management experience. You will follow established procedures for charting, narcotics handling, incident reporting, and rounds with physicians as needed.

Qualifications

• Long-term care experience required.
• Point Click Care charting experience required.
• Active LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions (nutrition, infections, skin conditions, weight loss, etc.).
• Uses the Quality Life Manual for patient identification, placement, and monitoring.
• Documents room changes, LOA, new admits, or discharges and notifies departments.
• Makes rounds with physicians when necessary.

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
