Job summary

Healthforce is seeking an experienced Licensed Practical Nurse (LPN) for a travel assignment in Rochester, New Hampshire. Start date 2026-10-05 for a 6-week duration, with guaranteed 40 hours per week and evening shifts from 3:00 PM to 11:30 PM in a long-term care nursing home.

Responsibilities include administering medications, monitoring vital signs, wound care, and collaborating with the RN and care team to ensure high-quality patient care.

Qualifications

• Active LPN license in good standing.
• Ability to work evenings as scheduled.
• Strong communication and teamwork skills.
• Experience in long-term care setting desirable.

Responsibilities

• Administer medications and treatments per physician orders, verify MARs, and document outcomes.
• Monitor residents’ vital signs, assess changes in condition, and report findings to the supervising nurse.
• Perform basic wound care, dressing changes, and support activities of daily living as needed.
• Maintain patient safety with fall precautions, infection control, and adherence to care plans.
• Collaborate with the RN and interdisciplinary team to coordinate patient care and communicate changes.
• Document nursing actions accurately in the electronic chart and ensure care plans are followed.
