Job summary

Healthforce in Bennington, VT is seeking a Licensed Practical Nurse for a 13-week travel assignment in a skilled nursing facility. The role is night shift 11:00 PM–7:30 AM with 40 guaranteed hours per week and a start date of 10/01/2026.

You will provide direct patient care, administer medications, monitor vitals, and participate in care plan coordination with the interdisciplinary team. Active LPN license and completion of an approved LPN program are required; weekends and long-term care

Qualifications

• Active LPN license in good standing
• Completion of an approved LPN program
• Prior experience in long-term care or skilled nursing settings preferred
• Ability to work night shifts and weekends as assigned
• Strong assessment, documentation, and communication skills
• Capable of functioning effectively in a fast-paced, team-based environment

Responsibilities

• Provide direct patient care to residents within the scope of an LPN, under supervising nurse
• Administer medications and document accurately in the resident chart and electronic health record
• Monitor vital signs, observe for changes in condition, and report findings to the RN or physician
• Assist with activities of daily living and support resident safety, mobility, and comfort
• Complete routine procedures within scope, including basic wound care and dressing changes
• Collaborate with an interdisciplinary team to coordinate care plans and ensure smooth shift handoffs
