Qualifications

• Graduate of an approved Practical Nursing program.
• Active LPN license in the State of Iowa (or a valid Compact/Multistate LPN license).
• Current Basic Life Support (BLS) / CPR certification.
• 1+ years of nursing experience in a long-term care or skilled nursing setting.
• Shift: Night shift 6:00 PM – 6:00 AM.
• Excellent communication with families and multidisciplinary team; proficient in EHR documentation.

Responsibilities

• Monitor vital signs, conduct routine assessments, and administer medications with EHR documentation.
• Perform wound care, dressing changes, and other routine procedures per care plans.
• Promptly communicate changes in resident condition to RNs, physicians, and resident families.
• Assist residents with activities of daily living (ADLs) and support rehabilitation activities as directed.
• Maintain accurate medical records and follow facility safety protocols.

• Daily Support: Assist residents with activities of daily living (ADLs) and support ongoing rehabilitation activities as directed


• Compliance & Safety: Maintain precise, timely medical records in the EHR and follow all facility safety protocols

Requirements & Qualifications
