HealthForce Healthcare seeks an experienced Travel Licensed Practical Nurse (LPN) for a Long-Term Care assignment in Dubuque, IA. You will deliver patient-centered care, manage chronic conditions, and support residents’ safety and dignity in a fast-paced setting.

Weekly pay is $1,378 and the role features a consistent Day Shift on a travel contract, with local support provided throughout the assignment.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current, unencumbered LPN license in the State of Iowa (or active Compact/NLC state license).
• Active Basic Life Support (BLS) / CPR certification.
• Minimum 1+ years of long-term care or skilled nursing facility experience.
• Strong verbal and written communication with healthcare teams and families; proficiency in EHR documentation.

Responsibilities

• Administer medications and monitor vital signs; perform wound care per care plan.
• Assess changes in resident health and report promptly to RNs, physicians, and families.
• Collaborate with CNAs, therapists, and healthcare teams to support ADLs and rehabilitation goals.
• Maintain accurate, timely resident records in the EHR per safety standards.

Key Responsibilities

• Resident Care & Monitoring: Administer medications, monitor vital signs, perform wound care, and execute routine nursing procedures following individualized care plans
• Assessment & Reporting: Continually assess changes in resident health conditions and promptly communicate updates to RNs, physicians, and resident families
• Collaboration & Support: Work alongside CNAs, therapists, and multidisciplinary healthcare teams to assist with activities of daily living (ADLs) and rehabilitation goals
• EHR Documentation: Maintain accurate, timely resident records within the Electronic Health Record (EHR) system in compliance with facility and safety standards

Qualifications & Requirements

Required

• Education: Graduate of an accredited Practical Nursing Program
