Job summary

HealthForce Healthcare seeks a Licensed Practical Nurse for a travel assignment in a long-term care with acute rehab focus in Vermont. You will cover evenings (3:00 PM–11:30 PM), 40 hours per week, for an 8-week period starting 11/02/2026.

Responsibilities include med administration, patient monitoring, ADL support, and interdisciplinary collaboration to meet rehab goals. Required VT LPN license, CPR/BLS, and previous LTC/acute rehab experience preferred.

Qualifications

• Current Vermont LPN license in good standing.
• CPR/BLS certification.
• Graduation from an approved practical nursing program.
• Ability to work evenings and participate in an eight-week assignment.
• Prior experience in long-term care or acute rehab settings preferred.
• Ability to comply with facility policies and report changes promptly.

Responsibilities

• Administer prescribed medications and treatments following physician orders.
• Monitor patient conditions, including vital signs, and escalation changes per protocol.
• Assist with activities of daily living and rehabilitation activities to support mobility and independence.
• Document care in patient records, ensure accurate charting and timely updates.
• Collaborate with the interdisciplinary team (nurses, therapists, dietary and care coordination) to support rehab goals.
• Maintain infection control standards and wound care as assigned, and respond to changes in patient status.
