Qualifications

Responsibilities

• Provide direct patient care to residents within the scope of an LPN in a long-term care setting
• Monitor vital signs, administer medications and treatments as prescribed, and document observations
• Collaborate with RNs, physicians, and the interdisciplinary team to implement and adjust care plans
• Document care activities accurately in the electronic health record and maintain resident confidentiality
• Assist with admissions, transfers, and discharges per facility policy and procedures
• Ensure patient safety, fall prevention, infection control, and comfort; respond promptly to call lights and changes in condition
