Responsibilities

• Provide direct, evidence-based nursing care to long-term care residents according to the plan of care.
• Administer medications and monitor for adverse effects, documenting administration accurately.
• Perform routine assessments, monitor resident status, and report significant changes to the nursing team.
• Collaborate with interdisciplinary colleagues to coordinate care, safety, and discharge planning as appropriate.
• Maintain patient safety and infection control practices; ensure accurate charting and documentation in the electronic health record.
• Respond promptly to resident needs and assist with activities of daily living as assigned.
