Qualifications

• Current LPN license in NH is required.

Responsibilities

• Provide direct nursing care in a long-term care setting per care plans under RN supervision.

Responsibilities

• Deliver direct nursing care to residents in a long-term care setting, following established care plans and under supervision of a registered nurse.
• Administer prescribed medications and treatments, document administration and resident response in the chart, and monitor for adverse effects.
• Assess residents’ condition through ongoing vital signs monitoring, noting changes in health status and reporting promptly to the charging nurse.
• Support activities of daily living (ADLs) such as mobility, hygiene, feeding, and assistance with routines to promote resident comfort and safety.
• Collaborate with the interdisciplinary team to adjust care plans based on resident needs, goals, and agreed-upon clinical outcomes.
• Ensure compliance with infection control practices, proper medication handling, safe device use, and accurate documentation to maintain quality and safety standards.

Requirements

• Current Licensed Practical Nurse license in New Hampshire
• Completion of an approved LPN program (diploma or associate degree)
• Ability to work a 3x12 night shift in a long-term care setting
• Basic Life Support (BLS) certification current
• Prior long-term care experience preferred
• Strong communication skills and ability to document clearly and concisely
